Our Glendale Office at Abrazo Arrowhead Campus

Dr. MacKenzie sees patients at Arizona Sports Medicine Center at Abrazo Arrowhead Campus, located just off Loop 101 at West Union Hills Drive in Glendale. The office is convenient to Arrowhead Ranch, Peoria, Surprise, Sun City, Sun City West, Anthem, and North Phoenix, and patients drive in from across the West Valley. Free parking is available at Building B.

Any imaging (X-ray, MRI, CT) related to your problem, or the name of the facility where it was done.
A list of treatments you have already tried, including therapy, injections, or prior surgery.
Your referral, if your insurance plan requires one.

Second Opinions Welcome

If you have been told you need shoulder, knee, or hip replacement surgery, a second opinion is a reasonable step, not an insult to your first surgeon. Bring your imaging and Dr. MacKenzie will review it with you directly, explain what he sees, and give you his honest recommendation, whether or not that recommendation is surgery with him.
